Laparoscopic Sleeve Gastrectomy (also known as a Laparoscopic Tube Gastrectomy)

This is a restrictive procedure. The stomach volume is reduced by stapling it along its entire length. The final result is an approximately 125 mls stomach tube. The reduction in stomach volume restricts the amount of food able to be ingested at any one time, typically to the size of a dessert plate.

Of particular note, there is marked reduction in hunger and so one actually feels like eating less. The mechanism of the reduction in hunger is complex and in essence is thought in part to be due to the marked reduction in ghrelin, the hunger producing hormone (as the portion of the stomach that is removed contains ghrelin producing cells).

This procedure is done laparoscopically (via keyhole surgery) and is performed through five incisions measuring 5mm to 15mm. The length of the hospital stay averages 2 nights.

Gastrectomy
Advantages of Laparoscopic Sleeve Gastrectomy
  1. 1. Reduction of the amount of food without affecting the type of food

  2. 2. Normal stomach emptying

  3. 3. No malabsorption

  4. 4. No adjustments required post operatively, compared to the Gastric Banding

  5. 5. Ideal for patients in remote areas

Disadvantages of Laparoscopic Sleeve Gastrectomy:
  1. 1. Possible long-term weight regain if the tube stretches

  2. 2. An additional procedure is occasionally necessary later if sufficient weight loss is not obtained (or if there is weight regain)

In properly chosen patients, with this operation, typically 70 to 80% of excess weight loss is achieved and this weight loss is maintained for at least five years.
